From 2004 to 2016, under-five mortality has decreased in Tanzania, but there has been no change in neonatal mortality. Hypothermia due to prematurity and low birth weight, birth asphyxia and neonatal infections account for 40% of neonatal deaths in the country. Hypothermia is a medical emergency that occurs when the body loses heat faster than it can produce, causing low body temperature incompatible with life. About 57% of neonates in the entire East African region are hypothermic.
Many factors contribute to this challenge, including:
Environmental factors such as the temperature in newborn try care areas and remoteness of health facilities
Social factors such as low level of health education among members of the society
Financial factors. There are no sufficient funds to buy medical tools and equipment for health facilities.
In 2019, 74878 newborns died in the entire country. The tools (Modified incubator and Embrace nest) will eventually support 57.2% of neonates in the East Africa Region who suffer from hypothermia and save their lives.
The modified incubator is made mainly from locally available materials. It keeps the baby warm and free from environmental contaminants. It contains sensors to control and prevent temperature and humidity.
Embrace nest is used during a referral to a more advanced health facility. It is locally made. It keeps the baby warm for seven (7) hours while being transferred to other facility and in neonatal intensive care units.
Angle mark is used in newborns with asphyxia. It is locally made. It is used to position the baby's head during the management of asphyxia .
Value proposition for Key Stakeholders
The products (modified incubator, angle mark and embrace nest) address all their pains and will help them do work effectively and efficiently while reducing and avoiding newborn deaths .

Insight and Problem Framing
Neonatal disorders are the leading cause of all deaths worldwide. Hypothermia due to prematurity and low birth weight, birth asphyxia and neonatal infections account for 40% of newborn deaths in Tanzania. Hypothermia is a medical emergency that occurs when the body loses heat faster than it can produce, causing low body temperature incompatible with life. About 57% of neonates in the entire East African region are hypothermic.
Contributing factors:
Environmental: Remoteness of health facilities
Social factor: level of health education among members of the society
Financial factors. No sufficient funds to buy medical tools and equipment for health facilities.
Value proposition for Key Stakeholders
Our customers are the health care workers in primary health care facilities caring for newborns who are born prematurely, asphyxiated, with infections and, with congenital anomalies. Their major pains include death of the newborns due to hypothermia and birth asphyxia which may lead them to be depressed and even dissatisfied with their job.
All of our tools (Modified incubators, embrace nest and angle mark) contain mattress and/or nylon materials. These substances are hazardous to the environment. We will put in place a system to collect them for proper disposal and recycling. However, those instruments once in a health facility, they last for a very long time before disposal.
Internal Impact
At the start, the company will have a small number of employees. However, as we expand, we expect to have an increased number of employees. We will focus on in job-training to build individual capacity. For our company to flourish, we will be:
Disruptive and realistic in setting strategies
Action-oriented and Reflective in leading activities
Directive and inclusive in leading our employees
Goal-oriented and process-oriented
System Change
Since our products will be locally available and cheaper than the imported ones and given the fact that no neonate has died since we started to use them at Mererani Health Centre, we expect that the governments will emphasize on their uses and even influencing legislation. In that way, the tools will dramatically reduce neonatal and child mortality locally and internationally, especially in the sub-Saharan region.
Solution summary
Atomatic modified neonatal incubator to preventing temperatures drop down of the new born baby
Embrace nest: To keep the baby warm in the movements within the health facility and during a referral to a more advanced health facility.
Angle mark: Helps in positioning the baby's head in the management of asphyxia

Current State Stakeholder System
In Tanzania, the main stakeholders are the government and the non-government sector that include non-government organizations and the private health sector. The Ministry of Health, Community Development, Gender, Elderly and Children (MoHCDEC) is responsible for policy-making. In contrast, the President's Office Regional Administration and Local Government (PO-RALG) implements such policies. All the money for health-related activities from the Ministry of Finance and Planning is deposited directly to the public health facilities bank accounts. The money sent depends on the budget that is previously set by health facility governing committees. The committee decides on how to spend the money, including the decision to obtain essential health equipment. Non-government health facilities decide independently depending on their revenues and on the number of patients they attend. Local organizations such as CCBRT support other health facilities to provide better care to prevent disabilities. International organizations such as UNICEF provide support in knowing the pressing issues. Public and private research institutions support in giving scientific evidence on what is working.
The early adopters are the public and non-public primary health facilities (dispensaries and health centers). We will reach them through the following channels:
Direct contact with health facilities and the District Medical Offices
We will engage with responsible government bodies such as the MoHCDEC and PO-RALG and non-government organizations such as Save th thise Children about the products.
We will also use telecommunication media to broadcast and making awareness of the tools to serve the lives of babies from hypothermia and birth asphyxia.

The target for our solution is the primary healthcare sector in developing countries, especially sub-Saharan Africa. The Primary healthcare sector includes dispensaries and health centers. As a starting point, we aim to reach 30-40% of primary healthcare facilities in Tanzania in the first five years. After that, to get the tools to the rest 60% in five years more. There are a total of 7731 facilities, 84% being dispensaries.
Sales, Marketing and Distribution Approach
Our startup will use the market-driven approach. We believe that our users' opinions and suggestions are fundamental in making sure that our products save and serve neonates, thus sustainably reducing morbidity and mortality.
Creating awareness of our products: Our startup will use word of mouth in our networks to spread the news about our homegrown solution. Besides, we are in a position to contact all health facilities' leaders.
Distribution: Our startup is expected to start its operations in Tanzania. Most regions are reachable through tarmac roads. Therefore, from the production point, we will transport the goods using public or private transporters to reach our customers (Primary healthcare facilities). The sales personnel will establish prior communication with the buyer to coordinate the sending and receiving of the products.
On the other hand, when the company grows, we expect to have regional or zonal distribution agents. These agents will be buying our products at wholesale prices.
At present, to deliver one unit to the end-user may cost up to 10% of the selling price.
Customer Acquisition and Retention Strategy
Acquisition: A contacts database of primary healthcare facilities' leaders will be created and updated regularly. We will be contacting them and provide them with information about our products.
Activation: We will draw on the statistics of neonatal mortality and morbidity in the health facility, and we will explain how our solution can reduce the burden.
Conversion: Reduction of neonatal and maternal deaths in Tanzania is one of the significant health agenda, and all health facilities are required to follow and engage in solutions to save lives actively. Each health facility receives fund from the central government and has to plan its use depending on the needs of the facility. We will therefore use this chance to convince and make them understand that buying the equipment and using them correctly, will improve their statistics and thus being an example to the rest.
Retention:
We will establish automatic communication via email to the health facility to remind and refresh their knowledge on how to use the tools.
We will join forces with other actors in the field to train health care workers on the basics of neonatal management
