St. John’s National Academy of Health Sciences (SJNAHS) is a self-financing private medical Institution. St. John’s Medical College, Bengaluru established in 1963 by the CBCI with the aim of training healthcare personnel committed to serving the poor in the margins.
I am MD Psychiatry resident in St. John's Medical College and Hospital, Bangalore who developed this project called TRUST mobile health application in coordination with department of Psychiatry and Department of Gastroenterology to help patients diagnosed with alcohol dependence syndrome.
The harmful use of alcohol poses a significant global health challenge, contributing to millions of deaths and disabilities annually. According to the World Health Organization (WHO), alcohol consumption leads to approximately 3 million deaths each year worldwide and accounts for 5.1% of the global burden of disease. This burden disproportionately affects disadvantaged and vulnerable populations, exacerbating health inequalities. In India, specifically, where there are 16 crore alcohol users, 5.7 crore are problem users, and 2.9 crores are dependent users, the impact of alcohol-related harm is particularly pronounced.
The scale of the problem is immense, with alcohol being the leading risk factor for premature mortality and disability among individuals aged 15 to 49 years, responsible for 10% of all deaths in this age group. The consequences of harmful alcohol use extend beyond individual health outcomes, affecting families, communities, and societies at large.
Factors contributing to the problem include social and cultural norms surrounding alcohol consumption, limited access to effective interventions and support services, stigma associated with seeking help for alcohol use disorder (AUD), and inadequate awareness about the risks of alcohol misuse.
Our solution, the TRUST (Tracking Recovery, Urge Support, and Tools) application, addresses these challenges by providing personalized digital support to individuals with AUD. By offering features such as personalized recovery plans, real-time tracking of progress, stress management tools, and access to educational resources, TRUST empowers individuals to overcome AUD and maintain sobriety. This targeted intervention aims to reduce the burden of alcohol-related harm by improving access to evidence-based support and fostering a supportive environment for recovery.

We selected the Pilot stage of the project to validate the effectiveness and feasibility of the TRUST application in a real-world setting. So far, we have developed and tested the TRUST application, designed to assist individuals with alcohol use disorder (AUD) in monitoring and preventing relapse. The application includes features such as personalized recovery plans, daily tracking, stress management tools, and access to psychoeducational content.
Currently, we are conducting a randomized clinical trial study involving 40 patients diagnosed with AUD after approval from institutional ethics committee. These patients are being recruited from the Gastroenterology ward of our hospital. The study involves training patients to use the TRUST application as part of their inpatient treatment program. We are providing them with guidance on utilizing the application's functionalities and setting up personalized recovery plans.
After the initial intervention, patients are encouraged to use the TRUST application regularly. We are conducting follow-up assessments at one month and three months post-intervention to evaluate the impact of the application on patient outcomes, including relapse rates, mental well-being, and engagement in treatment.
Through this pilot study, we aim to gather data on the usability, acceptance, and effectiveness of the TRUST application in supporting individuals with AUD. These insights will inform further iterations and improvements to the application as we progress towards broader implementation.
We are applying to Solve because we believe it provides a unique opportunity for us to overcome specific barriers and accelerate the impact of our solution. As a team working on addressing the challenge of alcohol use disorder (AUD) through the TRUST application, we face several barriers that we hope Solve can help us overcome.
Firstly, we require additional funding beyond our current resources to further develop and scale our solution. 1. Increase Access to Quality Care: Our primary impact goal is to increase access to quality care for individuals with AUD, particularly those in underserved and low socioeconomic communities. To measure progress towards this goal, we track the number of downloads and active users of the TRUST application, with a focus on reaching target demographics in both urban and rural areas.
2. Reduce Relapse Rates: We aim to reduce relapse rates among users of the TRUST application by providing personalized support, evidence-based interventions, and continuous monitoring. To measure progress towards this goal, we track self-reported relapse incidents through in-app surveys, and online and offline interviews.
3. Assessment and Monitoring of craving for substance using standardized scales like The Penn Alcohol Craving Scale (PACS) which is a 5-item questionnaire that measures an individual’s craving to drink alcohol in the past week. The PACS includes questions about the frequency, intensity, and duration of craving, the ability to resist drinking, and an overall rating of craving for alcohol for the previous week.
4. Assessment and Monitoring of severity of alcohol withdrawal symptoms using CIWA-Ar Scale, also known as the Clinical Institute Withdrawal Assessment for Alcohol–Revised. It consists of a 10-item scale that evaluates various symptoms such as nausea, tremors, and anxiety.
5. Regular monitoring and measure the level of dependence using The Severity of Alcohol Dependence Questionnaire (SADQ or SAD-Q) which is a 20 item clinical screening tool divided into five sections: Physical withdrawal symptoms, Affective withdrawal symptoms, Craving and relief drinking Typical daily consumption and Reinstatement of dependence after a period of abstinence. Some local clinical guidelines use the SADQ to predict the levels of medication needed during alcohol detoxification.
6. Enhance Quality of Life: We aim to enhance the overall quality of life for individuals with AUD by promoting sobriety, increasing social support, and improving functional outcomes such as employment and relationships. To measure progress towards this goal, we track changes in self-reported quality of life indicators, including measures of physical health, emotional well-being, social functioning, and overall life satisfaction.
7. Reduce Healthcare Costs: We seek to reduce healthcare costs associated with AUD by preventing relapse, decreasing hospitalizations, and improving overall health outcomes. To measure progress towards this goal, we analyse healthcare utilization data, including hospital admissions, emergency room visits, and outpatient services, among TRUST users compared to control groups receiving standard care.

The TRUST application is a user-friendly progressive web application (PWA) which is a type of application software delivered through the web, built using common web technologies including HTML, CSS, JavaScript, and Web Assembly. It is intended to work on any platform with a standards-compliant browser, including desktop and mobile devices.

a. Responsiveness: PWAs are designed to adapt to different screen sizes and devices, providing a seamless user experience across various platforms.
b. Offline capabilities: PWAs can work offline or on low-quality internet connections, thanks to service workers that cache data for later use.

Our team has been actively working on the TRUST (Tracking Recovery, Urge Support and Tools) solution for one year, during which time we have been conducting a clinical trial, app development, content development, brief intervention, institutional ethical clearance and to evaluate effectiveness and impact of mobile health application on individuals with alcohol use disorder (AUD). Currently the initial phase of trial is going on patients after their consent.

Evidence of our success in funding our work includes: I have received Advanced In- Country Research Award, under Psychiatry Research Infrastructure for Intervention and Implementation In India ( A Fogarty International Centre - NIH funded project) to do the research project. The study is also being presented at University of Pittsburgh, Symposium 'India Comes to the Veterans Administration: The impact of Global Mental Health on Veteran's Care'.
The study is also supported by St. John's research Institute, Bangalore for hiring and paying the salary for Junior Research Fellow. The department of Psychiatry and Department of Gastroenterology have given permission to use the resources without any charges to conduct the study with a common goal of helping the patients and innovate health care.
