I vividly recall the trauma I experienced upon learning of my aunt's diagnosis with Stage 4 breast cancer. Her cancer had metastasized to her lymph nodes, and the doctors in Karachi, our city, refused to administer chemotherapy, stating it would only inflict more pain on her. Her fate might have been different had she sought treatment for her symptoms 22 months earlier, when they first appeared. Tragically, my aunt passed away in less than twelve months.
This experience motivated me to pursue a bachelor's in Biomedical Sciences and piqued my interest in enrolling in a Cancer Biology course at my university in Canada. There, I focused on breast cancer statistics in Canada, particularly those concerning late detection. The alarming numbers in a developed country like Canada prompted me to consider the situation in developing countries, especially Pakistan, where my aunt succumbed to breast cancer.
Breast cancer constitutes 24.5% of all cancer cases globally. In Canada alone, breast cancer represents 25% of new cancer cases and approximately 14% of all cancer-related deaths. Despite the robust education system in Canada, I pondered the breast health knowledge gaps in markets like Pakistan. Even with Canada's healthcare system investing immense resources into reducing breast cancer mortality, the statistics remain distressing.
I questioned whether the situation was worse in Pakistan.
Indeed, it is! My aunt's experience is not an isolated case. Pakistan, the Asian country with the highest breast cancer prevalence, reveals that 1 in 9 women will develop breast cancer during their lifetime. Alarmingly, as of 2021, 89% of breast cancer patients in Pakistan are diagnosed at a later stage, with 59% at an advanced stage. To put things in perspective, in 2020 alone, there were 2.3 million women diagnosed with breast cancer and 685,000 deaths globally.
The reasons for these statistics are manifold. The subject of sex education is entirely excluded from our school curriculums, and discussions on breast health are stigmatized and regarded as taboo. Breasts are often viewed more as sexual symbols than health-related organs, perpetuating a myriad of misconceptions and stigmas. These range from the erroneous belief that breast cancer is contagious to deep-rooted fears of societal abandonment, especially from husbands post-diagnosis. Additionally, many young women hide their lumps out of fear that they will not find a suitor in an arranged marriage system.
Late detection is also frequently linked to women's hesitancy towards screening, fueled by the misconception that mammograms might compromise their morality. In such societies, it is often deemed immoral for women to undergo screenings or even discuss their breast health.I was particularly shaken by the story of a woman in Gurgaon, India, who committed suicide due to her breast cancer diagnosis, leaving behind a note clarifying that she blamed no one for her decision.
Our key product, PinkDetect, is an AI-driven app and a breast health management platform tailored for Pakistani women.







Designed with precision, it integrates distinct features to provide underserved Pakistani women with intuitive user experience:
Cultural Affinity in Content: Recognizing the nuances of our conservative target audience, the resource tab of the PinkDetect app meticulously curates culturally-sensitive materials about breast health and cancer. This encompasses animated step-by-step tutorials on breast self-examination (BSE), allowing users to educate themselves about its significance, execute it correctly, and discern potential anomalies for breast cancer that merit medical attention. Furthermore, it offers insights on signs, symptoms, risk factors, and preparatory guidelines for mammogram appointments in layman and non graphic terms/imagery to appeal to the Pakistani masses.
AI-Powered Risk Assessment: PinkDetect utilizes advanced AI algorithm to gather and analyze health data from the user, such as age, family history, weight, height, breast density, and any known genetic mutations. By processing this information, the AI tool can determine if the user is at a higher risk of developing breast cancer. If a higher risk is identified, the app will send more frequent reminders in the form of pop-up notifications to encourage regular self-examinations and mammograms.
Reminder System: In addition to the AI-driven personalized reminders, the app also sends periodic reminders to all users, regardless of their risk level, to emphasize the importance of breast self-examinations and early detection.
Location-Based Services: PinkDetect app features a built-in map locator that helps our remote Pakistani users, especially those residing in rural areas, find nearby mammogram services, healthcare practitioners, and clinics specializing in breast health. This feature aims to bridge the accessibility gap between awareness and access to healthcare services by instantly connecting users with the appropriate facilities in their area.
Journal Feature: The app includes a journaling function that allows users to document their symptoms, such as tenderness or changes in their breasts. This serves as a personal record that can help users track and monitor their breast health over time. It can also be valuable in providing relevant information to healthcare providers during consultations while ensuring even the most subtle changes are acknowledged by the user and don’t go unnoticed.

Our app boasts an innovative audio library, offering concise 2-3 minute voice snippets, articulated in accessible language devoid of technical jargon. These segments highlight the latest advancements in breast cancer research. Beyond mere education, PinkDetect aims to democratize relevant culturally inclusive breast health knowledge and actively dismantle prevailing stigmas and myths around the subject by addressing those faced by Pakistani girls and women.

Let me introduce you to Saima, a 48-year-old woman living in a rural town of Pakistan called Badin. Saima is a housewife and a mother of three, working tirelessly to provide for her family. Her understanding of breast cancer, including its risk factors and the critical importance of early detection, was minimal. This gap in knowledge is not unique to Saima; it reflects a broader issue among many Pakistani women, hindered by financial, cultural, and informational barriers in accessing healthcare and health information.
One day, noticing a yellowish discharge from her breast, Saima's concerns were silenced by the weight of cultural stigma and fear. The absence of knowledge and open discussion delayed her medical consultation significantly. By the time she mustered the strength to seek help because she could not manage the pain in her breasts anymore, she was diagnosed with Stage 3 breast cancer, now metastatic, cornering her into limited and financially burdensome treatment options.
Saima’s story is a vivid illustration of the challenges many Pakistani women face, spanning urban to rural areas and varying socio-economic statuses. These women share a common thread: a gap in breast cancer knowledge and the consequential challenges in accessing healthcare, compounded by financial constraints, socio-cultural stigmas, geographical barriers, and a lack of awareness about available resources.
In Pakistan, a country home to over 110 million women who are often excluded in HealthTech advancements, PinkDetect app emerges as a transformative tool in a landscape where breast cancer awareness and early detection are alarmingly low. PinkDetect is not just an app; it's a comprehensive breast health management platform, offering culturally sensitive education, guiding women through self-examinations, and providing accessible information about breast cancer. Designed to reach women regardless of their location or socio-economic status, PinkDetect challenges cultural stigmas and promotes open dialogue about breast health.
By democratizing breast health knowledge, PinkDetect aims to empower Pakistani women, propelling awareness, early detection, and timely medical consultations. Our PinkDetect app is currently in the prototype stage, where we have developed both a Figma prototype showcasing an intuitive and culturally sensitive interface for breast health management and completed the front and back-end coding. We began testing our PinkDetect app by integrating the testing sessions in our grassroots initiatives around breast cancer awareness.
We initiated our app testing with youth, recognizing that empowering and engaging the younger generation is pivotal for a paradigm shift in health literacy and breast health conversations. Our first testing session was conducted at Jesus and Mary College in India, partnering with the Peace Society. Although our primary focus is Pakistan, this remote workshop in India was chosen due to cultural similarities and our eventual plans to scale to India, Bangladesh, and the global south. The workshop attracted over 200 participants, marking a significant step in our breast cancer advocacy. One of our team members, Dr. Sana Shaikh, led a presentation on breast cancer prevalence and the importance of self-examination, followed by the showcasing of PinkDetect’s animated cartoon on conducting a breast self-exam.We personally observed the reactions of our attendees as they watched the video. Far from shying away due to the topic's taboo nature, the girls were significantly engaged. One participant even suggested that we dub the same cartoon in Urdu/Hindi so she could share it with her mother and grandmother, who do not understand English. This session was followed by our app's testing, where our audience not only tested the PinkDetect app but also provided insights and feedback, crucial for refining our app UI. They expressed a desire for more information on mammograms, leading us to enhance our inbuilt app resource library with detailed information about mammograms, including what they are and how to prepare for one.
Following the success of our first workshop and app testing session, we expanded our outreach to another university, this time in Pakistan. Upon launching PinkDetect, we had recognized the critical importance of engaging men, particularly young men, in our breast cancer advocacy, despite initial uncertainties about initiating this conversation. In Pakistan, where discussing breast health is often taboo, men's support can significantly encourage women to proactively manage their health. Thus, we organized a breast cancer awareness workshop at the Institute of Business Administration in November, making a concerted effort to include men, reinforcing the message that discussions about breast health should be ongoing throughout the year. We proudly presented our animated tutorial on breast self-examination and introduced our app prototype for testing, gathering invaluable feedback. This workshop received immense support from the Pollination Project Grant, JS Bank, Future Trust Pakistan, and the Toronto Metropolitan Association of Part-time Students.
Encouraged by the engagement of the youth with PinkDetect, we sought to reach an older demographic in Pakistan. With support from the Western Union Foundation and the Watson Institute, we shifted our focus from workshops to medical camps in remote Pakistani settlements, where many women have access to affordable smartphones and mobile internet. In collaboration with the Humanity Initiative, the PinkDetect team was privileged to engage 120 remarkable women from Agra Taj Colony in Lyari. We provided essential breast health education and complimentary clinical breast exams. Through interactive sessions with our app prototype, digital content, and one-on-one consultations with our onsite physicians, we broke down barriers and equipped women with the knowledge and tools needed for improved healthcare access. It was gratifying to identify 4 early-stage tumors through our clinical breast exams, a vital achievement in our mission to save lives. These women were immediately referred for mammograms. Remarkably, 90% of the participants expressed their intention to download our app and use it for monitoring their self-exams.
Our impact analysis, based on before-and-after survey responses from the women attending our camp, revealed significant shifts in awareness and behavior:
- Initially, 85% of participants did not perceive themselves at risk for breast cancer; post-camp, this shifted to 100% recognizing that every woman has some level of risk.
- Remarkably, 87.5% of attendees had never participated in a breast health seminar before ours.
- A substantial 89.7% acquired new knowledge about breast cancer during the camp.
- 90.6% affirmed that the camp effectively addressed their health concerns.
- Confidence in performing breast self-exams (BSE) surged from 50% pre-camp to 97.5% post-camp.
- 90% of the women felt a digital solution like our app would empower them to holistically manage their breast health and adopt a proactive approach.
Our collaborator, the Humanity Initiative, played a crucial role in this camp's success and is keen to extend our efforts to other underserved communities. They also champion the integration of similar health education programs in schools and colleges, targeting young girls, to further spread essential health literacy.
We have already replicated this successful camp in Macchar Colony, empowering an additional 180 women with a carefully designed agenda to promote breast cancer awareness and app testing. Three more camps are scheduled, continuing our dedicated effort to combat breast cancer.
Our mission at PinkDetect is dedicated to empowering Pakistani women to take charge of their breast health, thereby aiming to reduce breast cancer mortality rates significantly. Within the next two years, we aim to extend our reach to 200,000 women who are often excluded in healthcare and technological advancements. Our strategy involves a tech-forward approach, incorporating remarkable AI features into our app:
Enabling users to upload images of symptoms such as lumps or discharge for analysis by our AI algorithm. Furthermore, we plan to enhance our algorithm to interpret MRI results, a critical advancement considering the lack of medical facilities and telemedicine in numerous regions across Pakistan.
Achieving the aformentioned ambitious milestones requires not only funding but also comprehensive tech mentorship and ethical AI guidance.
We recognize the underdeveloped state of AI innovation in Pakistan and see MIT Solve as a vital ally in our endeavor. Renowned for its community of tech entrepreneurs and ethical AI pioneers, Solve offers the mentorship we need to refine our ethical AI practices and data collection methods. Although we have laid foundational work in these areas, the nuanced expertise within Solve's network can elevate our project's ethical framework and technological efficacy.
Our objective to positively affect 200,000 women demands widespread support and validation. Solve's endorsement and the network it provides could be pivotal, enabling us to forge partnerships with nonprofits, local government bodies, and key stakeholders in the healthcare sector, including hospitals and pharmaceutical companies. Our collaboration with MIT Solver Saba Khalid, founder of Aurat Raaj, has already shed light on potential paths for collaboration and scalable data collection. Her mentorship, drawn from her experiences and connections established through Solve, has been instrumental in engaging with local government and healthcare stakeholders in Pakistan to further our advocacy on breast health. We are keen to broaden our network, particularly aiming to engage with more MIT Solvers like Naba Khalid, co-founder of Poket, to explore effective strategies for scaling our community-led data collection and breaking into communities that might otherwise remain beyond our reach.

Furthermore, the app facilitates real-time connections with nearby screening clinics, effectively bridging significant accessibility gaps and ensuring that crucial health services are within reach, especially for our rural users.

Our inaugural workshop involved 200+ participants, who then pledged to advocate for regular breast exams within their households. Moreover, our first medical camp in Karachi offered breast health education and free clinical breast exams to 120 women, identifying 4 early-stage tumors, which spurred us to extend our reach to similarly affected areas. The following statistics further illustrate our camp’s success:
- Before the camp, 85% of women believed they were not at risk of breast cancer. After, 100% understood that every woman holds some risk.
- 87.5% had never attended a breast health session.
- 89.7% of women gained new knowledge about breast cancer from our camp.

App Enhancement: We conducted in-depth focus groups to gauge the extent of the problem. One of them revealed a compelling story of a woman who traveled 200 kilometers from Badin to Karachi for a breast exam, unaware of closer healthcare facilities. This discovery led to the development of an in-app map locator, connecting women with nearby screening centers, addressing geographical barriers.
These interventions helped us establish our strategic vision, with the following long-term outcomes:

According to the American Cancer Society, early detection significantly increases survival rates, with the 5-year survival rate for localized breast cancer reaching 99% when detected early. By promoting regular checks, we anticipate not only improvements in individual breast health management but also a broader economic impact, as early detection can substantially lower healthcare costs by reducing the need for more extensive treatments at later stages. This is especially critical in Pakistan, where healthcare resources are limited and the economic burden of late-stage cancer treatment can be overwhelming.
Our vision has attracted support from organizations such as JS Bank, the Pollination Project, Future Trust, Watson Institute, and the Western Union Foundation. Our partnership with Jinnah Laboratory, offering PinkDetect users discounted rates on medical tests, further encourages proactive health management. By synthesizing our targeted activities with anticipated outcomes, PinkDetect is poised to redefine breast health awareness, detection, and care. Ultimately, we aim to expand to India and Bangladesh, driven by the universal need for culturally-sensitive breast health education.





At PinkDetect, our primary aim is to notably reduce the incidence of late-stage breast cancer diagnoses. With a target of 20,000 app downloads in the initial four months post-launch, we strive for at least 50% of our users to consistently log their self-exams bi-weekly. Given that 1 in 9 women in Pakistan are at risk of developing breast cancer in their lifetime, our focused efforts are expected to lead to the early detection of approximately 1,000 tumors. In partnership with healthcare entities like Jinnah Laboratory, which offers our users a 20% discount on mammograms and CT scans, we anticipate facilitating around 500 mammograms for those detected in early stages. This endeavour not only furthers our commitment to timely healthcare interventions but also aims at measuring the economic benefits derived from early detection, focusing on the cost savings from less intensive treatments and the accelerated capability for women to return to their daily lives including work.
As we expand our grassroots initiatives with a goal to educate 100,000 women across Pakistan through workshops and medical camps, our collaborations with organizations such as the Humanity Initiative and JS Bank play a pivotal role. Events like our upcoming camp at Ziauddin Hospital, poised to impact an additional 150 women, are central to our mission. We utilize pre and post-event surveys to gauge enhancements in breast cancer knowledge among participants, ensuring our educational endeavours yield tangible improvements and impact in awareness and early detection.
To improve accessibility to services, we monitor the uptick in screenings facilitated by our app's map locator feature and our strong partnerships with healthcare providers. These metrics are crucial in highlighting our strides towards making breast health services more accessible, particularly in underserved communities. Through qualitative feedback gathered from community engagement activities, we're also assessing shifts in societal attitudes towards breast health, with a keen eye on the increasing participation of men in our educational and early detection initiatives as an indicator of a broader societal acceptance and support for breast health awareness.

This engine, leveraging models like logistic regression or convolutional neural networks, also integrates the Gail Model to refine our users' risk assessments. Further innovation includes a GPT-4 powered chatbot and Large Language Models (LLM) in our premium version of the app, crafted to provide our users with empathetic, culturally attuned breast health information in real-time.
We're conducting research on an AI algorithm, in collaboration with our interns from Canada and Pakistan who have academic backgrounds in biomedical engineering and computer science. This algorithm is designed to detect breast cancer symptoms through user-uploaded images. Additionally, we're exploring capabilities to enable our AI to analyze mammograms or CT scans, distinguishing between benign and malignant tumors, thereby setting new benchmarks in diagnostic technology.
The PinkDetect app, developed with Figma and powered by Node.js and Python, is the tech-savvy user's gateway to our innovations, featuring multilingual support to transcend language barriers. It boasts animated tutorials for breast self-exams and an 'Audiopedia' that demystifies breast cancer knowledge for varying literacy levels. A GPS-based map locator further dismantles access barriers by linking users to nearby screening centers.
