I vividly recall the trauma my best friend and co-founder of PinkDetect, Suha, feels when she remembers her aunt's diagnosis with Stage 4 breast cancer. Her aunt's fate might have been different had she addressed her symptoms 22 months earlier when they first manifested. Suha, also my classmate, and I later took a Cancer Biology course at our university where we focused on breast cancer statistics in Canada, particularly those concerning late detection. We were taken aback by the concerning numbers in a developed country like Canada, prompting us to consider the situation in developing countries, especially Pakistan – Suha's homeland and where her aunt succumbed to breast cancer.
Breast cancer accounts for 24.5% of all cancer cases on a global scale. In Canada alone, Breast cancer represents 25% of new cases of cancer and approximately 14% of all cancer related deaths. Considering the robust education system in Canada, we pondered the breast health knowledge gaps in frontier markets like Pakistan. Despite Canada's healthcare system pouring immense resources into reducing breast cancer mortality, the statistics remain alarming.
We questioned: is the situation worse in Pakistan?
Indeed it is! Suha's aunt's experience is not an isolated case. Pakistan, the Asian country with the highest breast cancer prevalence, reveals that 1 in 9 women will develop breast cancer during their lifetime. Alarmingly, as of 2021, 89% of breast cancer patients in Pakistan are diagnosed at a later stage, with 59% at an advanced stage. To put things in perspective, in 2020 alone, there were 2.3 million women diagnosed with breast cancer and 685,000 deaths globally.
The aforementioned statistics primarily stem from a plethora of reasons. The subject of sex education is entirely excluded in school curriculums, and discussions on breast health are stigmatized and regarded as taboo. While breasts are often viewed more as sexual symbols than health-related organs, a myriad of misconceptions and stigmas persist. These range from the erroneous belief that breast cancer is contagious to deep-rooted fears of societal abandonment, especially from husbands post-diagnosis. Additionally, we discovered that many young women hide their lumps out of fear that they would not find a suitor in an arranged marriage system.
Late detection is also frequently linked to women's hesitancy towards screening, fueled by the misconception that mammograms might compromise their morality. In such societies, it's often deemed immoral for women to undergo screenings or even discuss their breast health. We were particularly shaken by the story of a woman in Gurgaon, India, who committed suicide due to her breast cancer diagnosis, leaving behind a note clarifying that she blamed no one for her decision.
Our flagship product, PinkDetect, is an AI-driven app and a breast health management platform tailored for Pakistani women. Designed with precision, it integrates distinct features to provide an unparalleled and intuitive user experience:
Cultural Affinity in Content: Recognizing the nuances of our conservative target audience, the resource tab of the PinkDetect app meticulously curates culturally-sensitive materials about breast health and cancer. This encompasses an animated step-by-step tutorial on breast self-examination (BSE), allowing users to educate themselves about its significance, execute it correctly, and discern potential anomalies that merit medical attention. Furthermore, it offers insights on signs, symptoms, risk factors, and preparatory guidelines for mammogram appointments in layman and non graphic terms/imagery to appeal to the Pakistani masses.
AI-Powered Risk Assessment: PinkDetect utilizes an advanced AI tool to gather and analyze data from the user, such as age, family history, weight, height, breast density, and any known genetic mutations. By processing this information, the AI tool can determine if the user is at a higher risk of developing breast cancer. If a higher risk is identified, the app will send more frequent reminders in the form of pop-up notifications to encourage regular self-examination.
Reminder System: In addition to the AI-driven personalized reminders, the app also sends periodic reminders to all users, regardless of their risk level, to emphasize the importance of breast self-examination and early detection.
Location-Based Services: PinkDetect app also features a built-in locator that helps users find nearby mammogram services, family physicians, healthcare practitioners, or clinics specializing in breast health. This function aims to bridge the gap between awareness and access to healthcare services by connecting users with the appropriate facilities in their area.
Journal Feature: The app includes a journaling function that allows users to document their symptoms, such as tenderness or changes in their breasts. This feature serves as a personal record that can help users track and monitor their breast health over time. It can also be valuable in providing relevant information to healthcare providers during consultations. This ensures even the most subtle changes are acknowledged by the user and don’t go unnoticed.
Audiopedia: Our app boasts an innovative audio library, offering concise 2-3 minute voice snippets, articulated in accessible language devoid of technical jargon. These segments highlight the latest advancements in breast cancer research. Beyond mere education, this feature aims to democratize relevant culturally inclusive breast health knowledge and actively dismantle prevailing stigmas and myths around the subject by addressing those faced by Pakistani girls and women.

Let me introduce you to Saima, a 48-year-old woman living in an urban city of Pakistan, Multan. Saima is a mother of three, working tirelessly to provide for her family. She had limited knowledge about breast cancer, its risk factors, and the importance of early detection. Like many women in her community, Saima faced challenges in accessing healthcare services due to financial constraints, socio-cultural barriers, and a lack of awareness about available resources.
One day, Saima noticed a yellow-ish discharge in her breast, but due to cultural stigma and lack of knowledge, she hesitated to discuss her concerns with her family or seek medical help. Months passed, and Saima's health deteriorated. By the time she finally mustered the courage to visit a doctor, she was diagnosed with Stage 3 invasive ductal carcinoma. Her cancer had advanced significantly, and her treatment options were limited.
Saima's story is a stark reminder of the challenges faced by many women in Pakistan. Our target population includes women like Saima from diverse backgrounds, such as urban, semiurban and even rural areas, various socio-economic statuses, and different age groups. These women may have limited knowledge about breast cancer, its risk factors, and the importance of early detection. They might also experience challenges in accessing healthcare services due to financial constraints, geographical barriers, or lack of awareness about available resources.
In Pakistan, a country with over 110 million women as of 2023, our solution, the PinkDetect app, primarily serves women and girls where breast cancer awareness and early detection rates remain alarmingly low. These populations often miss out on comprehensive sex education and face limited access to healthcare services, inadequate breast health education, cultural barriers, and stigma around discussing and screening for breast cancer.
PinkDetect is designed to empower Pakistani women with crucial breast health knowledge and early detection tools. The app offers culturally sensitive education, guiding women through self-examination techniques and providing easy-to-understand information on breast cancer. With its inclusive approach, PinkDetect reaches women from urban to rural areas, ensuring that geographical or socio-economic barriers don't hinder access. Furthermore, by democratizing breast health knowledge, the app aims to challenge cultural stigmas and promote open dialogue. Ultimately, PinkDetect's goal is to proactively protect women by fostering awareness, early detection, and timely medical consultations, ultimately reducing advanced breast cancer diagnoses in Pakistan.

Our team understands the breast-health challenges our communities grapple with. Through surveys, focus groups, and workshops, we've ensured PinkDetect's implementation is firmly rooted in the community's expressed needs and aspirations. Our workshops relay breast cancer statistics, risk reduction strategies, and proudly showcase our cartoon on breast self-exam. Moreover, they serve as an essential platform for garnering feedback on the UX/UI of our app prototype. Our inaugural workshop, conducted in collaboration with Jesus and Mary College in India, saw an active participation of 300 individuals. From ideation to execution, we’ve prioritized voices of the community, making them co-authors in our shared journey towards breast health awareness.

Hailing from Pakistan, Suha possesses a profound understanding of the challenges that rural communities, particularly women, face in their daily lives.
Suha closely worked with an MIT-solver, Saba Khalid, and helped her deploy a chatbot in Pakistan aimed at educating women about menstrual-health. Immersing herself in the very heart of these communities, Suha also helped launch alternative sustainable menstrual pads designed by the ingenuity of rural artisans in Pakistan to bridge the menstrual product gap and provide women affected by inequality and lack of education with financial empowerment and economic stability.
Suha's advocacy for democratizing sex-ed has also been pivotal in pitching a webinar series exploring how COVID-19 impacted women's menstrual cycles. Eschewing the conventional digital-only medium, Suha helped broadcast these webinars directly in the homes of rural women, ensuring even those in the remotest areas were included.
Solmaz Ebrahimi Iranpour, Co-founder and Chief Research Officer
Solmaz hails from Iran, a country with complex socio-cultural dynamics affecting women's health. Her background in cancer research and her passion for understanding how inequalities affect marginalized communities, makes her an indispensable asset to PinkDetect.
The real eye-opener for Solmaz was the tragic incident of an Iranian woman being murdered for not wearing a hijab. This incident highlighted the overarching constraints placed on women's bodies.
Her involvement in the Iranian Donors Saves Lives campaign, aimed at bridging the gap in the Western Asian Iranian stem cell donor presence on the stem cell donor registry, ensure that PinkDetect isn't just a technological-solution but is imbued with empathy, understanding, and cultural-sensitivity.
Syed Waqas, Chief Medical Consultant
As a 4th-year medical student in Pakistan, during his clinical rotations, Waqas has been involved in multiple patient interactions, especially when it comes to inquiring about reproductive health. This reluctance is heightened when the medical practitioner is male, pointing to a deep-rooted societal stigma surrounding women's bodies.
Having been a part of numerous history-taking sessions, physical-examinations, and surgical observations, Waqas has witnessed firsthand the apprehension of women diagnosed with breast cancer. Pakistani women often delay medical intervention until their symptoms become undeniable. This delay is a combination of societal stigma and an inherent fear of the menacing nature of cancer. It's only through persistent probing that they are willing to vocalize their symptoms.

The stark reality in breast cancer care is the chasm between early and late detection—often the difference between survival and fatality. In Pakistan, research reveals that a staggering 60% of cases are diagnosed late, which tragically results in a 55-60% fatality rate. Compare this to the mere 10% mortality from early detection.
Annually, Pakistan witnesses about 40,000 breast cancer-related deaths, with women having an 11% lifetime incidence rate. Boosting early detection from 40% to 60% equates to an estimated 10,000 lives saved per annum. With a pragmatic goal of reaching a million women within three years (just 1% of women aged 15+), we anticipate facilitating early detection for about 1,500 cases—potentially sparing 1,000 lives. Yet, the real potency of our initiative lies beyond sheer numbers. A single educated woman can magnify this effect, becoming an advocate for self-examination within her community.
Early detection becomes increasingly important in communities with low-spend on education or those without insurance. For example, as per a study conducted by National Center for Biotechnology Information, in US, the average costs per patient in the year after diagnosis were $60,637, $82,121, $129,387, and $134,682 for disease stage 0, I/II, III, and IV, respectively. Accordingly, per patient, early detection (stage 0, I or II) results in an annual cost saving of roughly 50,000 - 70,000 in comparison to late detection in stage III or stage IV. This translates into savings of $ 50- 70 Million for the society if PinkDetect helps detect only 1,000 cases in early stages (these estimates are for US; the numbers will differ in Pakistan but the principle holds).
In the next 1 year, we want to formally launch our app. We are forging partnerships with local communities, healthcare providers, and NGOs to broaden our outreach. We will invest in awareness campaigns that speak to the heart of Pakistani culture, ensuring our message is widespread. To bolster our efforts, we're continually refining our app prototype, ensuring it's user-friendly and accessible. Moreover, we’re setting sights on expansion to regions that are underrepresented in health initiatives.
Focusing on digital platforms to disseminate educational content through workshops, and training sessions that are culturally relevant and easily understandable, we aim to educate women about the importance of early detection, techniques of self-exams, and when to seek medical advice. Our AI interventions ensure this newfound knowledge is personalized, offering users an intimate understanding of their unique risk landscape. Regular reminders foster a regimen of consistent self-examinations, while our location services and journaling feature respectively further access to healthcare facilities and personal health documentation.
These short-term outputs pave the way for significant outcomes. From early anomaly detection to a marked reduction in potential risks for high-risk individuals, the tangible benefits are several. Furthermore, with users better equipped to connect with healthcare services, there's a discernible rise in timely medical consultations.
