A customized blueprint in developing an efficient medical infrastructure for archipelago-structured regions.
Indonesia consists of approximately 17,000 dispersed islands, of which 70% are currently uninhabited. As the fourth most populous country in the world (with a population of around 250 million), the Indonesian government needs to overcome the challenge of providing equal welfare to its significant and growing population. Additionally, the task is further complicated by Indonesia’s archipelago nature, which results in a geographically- dispersed population for which it is difficult to achieve equal coverage of vital infrastructure such as transportation and healthcare services. As a result, Indonesians living in rural and remote areas face the following two problems: weak primary healthcare services, and almost non-existent secondary specialized care. Province of Maluku, also known as the province of a thousand islands, struggles with high maternal and infant mortality rates at 245 per 100,000 individuals and 60 per 1000 individuals, respectively. A data from a survey of 42,164 pregnant mothers showed that more than 35.3% of the mothers had suffered complications during pregnancy with over 70% of those mothers were left untreated post-delivery. 74.9% of those deliveries took place at home with 38.7% of the cases assisted by trained midwives and 1.2% (420 mothers) delivering their babies without any help. Only 19.5% of the mothers had access to post-delivery treatment. A low percentage (25.2%) of the new-borns were given exclusive breastfeeding and only 29.7% of them were immunized. An equally alarming data gathered from a sample of 179 children under five years from the province, showed that 28.3% of the children were malnourished and 40.6% of the children in the sample size had problems of stunted growth. Through providing specialized medical services such as obstetrics and gynaecology surgery as well as dental services, followed with empowerment programs for local healthcare providers and local people through a community-based approach, DoctorShare aims to empower individuals to attain basic health care so that they can exercise their right to live. Our floating hospital program exist not replace existing facilities provided by the Indonesian government, but instead as helping hand to ensure easier access and coverage for specialized care initiatives and to strengthen the existing primary care system provided by the government (in the form of Puskesmas and midwifery centers). Puskesmas are government-mandated community health clinics located across Indonesia. They are overseen by the Indonesian Ministry of Health and provide healthcare for population on sub-district level.

With over than 82% of Maluku territory being the Banda sea, the distance between islands make landed hospitals inefficient. Most villages are in remote areas that can only be visited through a difficult hiking trail or going up the stream, of which large navy vessels like the ones used by Mercy Ship cannot pass through. We noticed the corporation, Pertamina, was able to provide these villages with energy using barges, giving us the idea of creating a floating hospital barge. This new floating hospital model allow us to conduct major surgeries unlike previously in RSA1 and RSA2 (fishing boat).
One of the side project we are trying to achieve in DoctorShare is attaining a connected electronic medical records system among our different programs (floating hospital, flying doctors, Klinik K, and Klinik TB). With the implementation of this new system, we would be able to cut cost and become more sustainable by the reduction of the use of paper records. In addition, we would be able to speed up the process of providing our service.
I. Regarding our RSA3 Floating Hospital Barge, we hope that by the next 12 months we can have it running and connected to the Indonesian government BPJS program.
II. We can have our endowment fund program set up.
III. Implement more fluid restructuring in our organization in order for better and cost-saving operation, transparency, and global preparedness (easy fundraising flows for donators outside Indonesian, collaboration with international organizations, and accommodating foreign volunteers).
Aligning with our Founder’s, Dr. Lie Dharmawan, mission for DoctorShare, we believe that through a united collaboration amongst the youth, government, and organizations, we can empower our fellow brothers and sisters in remote areas of the world who have no access to basic health care. Through challenging the problem of inaccessible health care, we believe our floating hospital, flying doctors, and local clinic programs can be made into blueprints of which other organizations can adopt to help more people globally. Where organizations can run our blueprint in their local/targeted communities in a grassroot fashion.

RSA1 & 2: We dock in our targeted location and conduct regular check-ups,surgeries, and empowerment programs.
Flying Doctors: We fly our doctors and medical team as close as we can to the villages before hiking up to provide the local villages with check-up, surgeries, and empowerment program.
RSA3: We dock our barge to an island and conduct regular check-ups, surgeries, and empowerment programs. A drag-boat will pull the barge from island-to-island for a duration of 6 months before bringing the barge back for maintainance.
Klinik K and TB: Stationary clinic in Papua to treat malnutrition and Tuberculosis and empowerment programs.
Flying Doctors program: 145 minor surgeries, 1,165 recipients of health campaigns, 2,424 outpatient treatments/consultations.
Floating Hospital Boat (RSA 1 & 2): 744 Major Surgeries, 1,893 minor surgeries, 7,513 recipients of health campaigns, 910 USG & Antenatal consultations, 40,968 outpatient treatments/consultations.

The province Maluku has a population of 1.7 million accounted people and Papua has a population of 3.5 million accounted people. With the launch of RSA3 in Maluku, we predict a huge rise in medical services provided for the people of Maluku. We hope in 3 years we can convince the Indonesian government to adopt our floating hospital and/or flying doctor programs to be implemented in different areas of Indonesia that needs better medical infrastructure. And hopefully promote grassroot activities to increase the coverage of medical services in remote communities throughout Indonesia.

Although our overall team members have the medium age of 27. We are very passionate individuals who are led by Indonesia's key prominent cardiologist and social activist, Dr. Lie Dharmawan. Outside our operations team of pharmacists, doctors, surgeons, and individuals with hospital management background, we have highly connected individuals in our fundraising team to host high-return events ranging from USD $15,000 - 380,000 per event.

Firstly, the USD $650,000 will be used for either: 1) Covering our first year operation of our RSA 3 Floating Hospital Barge or 2) Starting our initial endowment fund program. Solve has access to MIT’s most brilliant minds who can provide us with expertise advice in mentoring us on how to effectively run DoctorShare's multiple programs. Eg. The chair, Peter Sands, to provide us advice for our Klinik K and Klinik TB programs to combat problems of malnutrition and Tuberculosis in Papua and other areas that we plan to set up clinics in collaboration with partnering organizations.
We have big dreams for our RSA3 Floating Hospital Barge program, unfortunately big dreams require big capital to get the project going. In addition, in order for the project to run smoothly, we need more talents joining our program who are able to join us full-time or at least senior mentors to guide us in terms of our organization strategy and expertise in setting up our endowment fund program, running our operation as a functional remote hospital, and attaining an efficient Medical Records System using basic technology that would function in both open waters and remote areas of East Indonesia.
