MBSProject reduces premature death from breast and cervical cancer in Colombian rural women through a mobile diagnostic unit and telemedicine

Bive is a social business that facilitates access to timely, high quality and affordable healthcare services for 19000 low income Colombian families.
Through a low-cost membership 19.000 people – 90% of them are coffee grower communities- access to private healthcare services, which are faster and of better quality than regular health services, at a price which they can afford, with a saving of up to 50% on the normal price.
Additionally, Bive develops social programs on health with its communities delivering health education and promptly diagnosis services in rural contexts. The Coffee smiles Project provided oral rehabilitation to 900 rural elders through dental prosthesis while the Coffee Vision Project has addressed the early detection and treatment of preventable causes of blindness for 2000 coffee growers.
Thanks to this experience, Bive has identified a new challenge: the need of addressing female health, 70% of breast cancer in Colombia is diagnosed on advanced clinical stages, 50% of symptomatic women take + than 3 months between the first consultation and initial treatment and only 42% of women from urban areas have adequate coverage of mammography and clinical breast screening, which is worse in rural areas where there is not diagnosis infrastructure and women’s are a community usually neglected in the highly manpowered Colombian rural context.
Mujer Bive tu salud (MBS) project facilitates access to an early detection and prevention of breast and cervical cancer in 200 rural women implementing a strategy based on point-of-care-testing model, telemedicine and a network of community health workers/ health providers to bring health services closer to rural women.
Through the development of MBS as pilot project Bive will bring closer female health services to 2 villages of the coffee region of Colombia by using a mobile health unit with capability of delivering mammogram screening, HPV test (Human papiloma virus) and pap smear with video-colposcopy and the usage of a software that integrates the whole health attention process and telediagnosis.
At the same time a family education program on health and self-care and healthy lifestyle for the prevention of cardiovascular diseases will be executed, supported by 4 female community health workers (CHW) that will be trained to improve auto efficacy, social participation and self-care levels in the target female population

- Effective and affordable healthcare services
Although Colombian health system provides breast cancer diagnosis services, it is hampered by long distances for rural communities (breast cancer diagnosis infraestructure is located in urban-areas) and long waiting times (4-6 months) due inefficient administrative procedures. MBSProject brings medical-professional team technological tools and a timely diagnosis (15 days) closer to isolated rural communities by a point-of-care-testing and telemedicine model based on a cloud platform that enables to specialized health professionals access to images taken in the field by the mobile diagnostic unit and a health education program delivered by community leaders. Technology enables to deliver inaccessible health care diagnosis services for rural, isolated communities in a cost-effective and sustainable way.
The Point-of-care-testing and telemedicine MBSProject model uses technology from the screening process (tank sample and images in field) to the delivery of diagnostic (remote connection with specialized healthcare professionals through the internet).
Likewise the educational program and social intervention are supported in communication technologies (SMS, whatsapp and internet) that connect Bive social time with community health workers.

Bive vision is to be the best healthcare solution for Colombia low-income rural communities based on education and technology for provide early diagnosis services for served communities and to scale-up health projects as MBSProject MBSProject vision is to be the model used by health system for the breast-cervical cancer detection and prevention in rural communities. With the experience of the piloting, learning outcomes will be shared with government and EPS-Insurance companies for its replication. Accordingly, Bive seeks to leverage investment in the future with other rural associations to contribute to the decrease of the morbidity-mortality of rural-women by cancer.

- Partnership with rural associations is a key successful factor for Bive scaling-up process and social projects development, which guarantees legitimacy and credibility inside communities. Bive has already partnered with 7 coffee growers cooperatives in two Colombia states to serve more than 18.000 coffee growers (Around 70% of total coffee growers families).

By the end of 2019, Bive will provide services for 23.000 people and it will serve 3.000 coffee grower with current social projects
Through MBSProject Bive will serve 200 rural women and their families, 4 community leaders and 10 healthcare-professionals
In 3 years Bive will be established in 10 Colombian states and it will reach 40.000 users. Likewise,MBSProject will deliver services for 5.000 rural women, 100 community leaders and 250 healthcare-professionals.
Bive final impact is to decrease the incidence of preventable diseases as cancer in rural communities through high-quality affordable healthcare services and health models based on education and technology.

Currently Bive is financial sustainable. Its expansion requires to increase of Bive membership sales through allies with rural associations. The development of social projects is part of the value proposition of Bive in the partnerships with rural farmers’ associations. Hence, MBS contributes strategically to the core-business-model and social impact vision.
MBS Pilot Project is not based on a self-sustainable model by itself, it will be cofunding through grants of international organizations and coffee grower cooperatives. Pilot experience will provide the platform for identify financial sustainable strategy as knowledge transmission model, partnership with government and healthcare insurances, partnership with fair trade certification companies (Partners of current coffee growers that are part of the project),etc.
