casenote
An online platform enabling continuity of care by providing affordable, easy access to healthcare & medical records using mobile devices.
1. Problems:

Bimbo*(L), a profusely respectful but disenchanted woman joins the search for Ola* her 2 year old son’s health records in a primary health center recently. Meanwhile a smart phone lies idle on the papers. *Not real names
- Inefficient method of collecting, storing, and retrieving health data in hospitals generally and primary healthcare centers specifically.
- Poor health-seeking behavior among our people.
- Inaccessibility to timely, relevant health information.
- High cost of treatment.
- Inefficient remote hospital management.
2. Solution:

casenote is designed to enable frontline health workers reach more people with their expertise and make informed clinical decisions by leveraging technology. The benefits of using casenote include: mobile access to patients’ health records, remote access to (specialized) health care or remote consultation for the frontline health worker thus improved access to healthcare, and optimized clinical decisions through artificial intelligence guided diagnosis and treatment, real time disease surveillance of their catchment area.
3. How casenote will change the world: Improve access to health care, drastically reduce mortality from medical errors resulting from inadequate medical history by making health records central and mobile as well as provide intelligent input into diagnosis and treatment. Improved patients engagement in healthcare. Reduce cost of healthcare attributed to complicated chronic conditions by early diagnosis and treatment. Early and easier disease surveillance for epidemics.

casenote is a new application of existing electronic health records technology. Its distinction is that health data are stored in a central database accessible to patients and across multiple caregivers using mobile devices as well as having integrated artificial intelligence feature to minimize medical errors.
The value in this model is the prevention of duplication of effort by frontline health workers managing a patient, cost efficient healthcare as well as improved clinical outcomes.
Technology is integral to our solution to achieve mobile proliferation of access points to healthcare and to create a care model that facilitates co-ordinated care across a critical mass of frontline health workers and facilities.
Technology is required to power our caseSearch service which enables real time disease surveillance and early detection of epidemic prone conditions.
Artificial intelligence is required to provide intelligent input on diagnosis and treatments to minimize mortality from medical errors attributed to misdiagnosis.
Sign up at least 2000 caregivers and reach at least 200,000 patients across communities in Africa.
Reduce mortality from medical errors attributable to inadequate medical history by 35-50% in the communities we serve.
Complete the ongoing process of securing partnership with the World Health Organization.
Leverage this partnership in selling our solution to governments who will in turn rope in players in their health ecosystem; for easy disease monitoring and contact tracing while sustaining the current features of mobile access to a central health records platform.

By leveraging on a partnership with the World Health Organization, we will sell our solution to governments who will bring their health industry players to adopt our solution as a standard of care.
Since launching 01-May-18 we have onboarded 3 hospitals in Lagos, Nigeria.
With our solution, caregivers have faster and easier access to patients health data; patients are more engaged in their care. Clinical errors due to disjointed health history has been effectively reduced or non existent.
In 12 Months: We expect that once we hit a critical mass of 10 caregivers, our database of caregivers and patients would grow geometrically.
So in 12 months we expect to have at least 2000 caregivers and a patient database of 200,000.
In 3 years: We expect to have close to a million patients if not more on our platform.
Services: We would serve them by providing mobile access to their health records with technological improvements in our featured services; caseChat, caseSearch. We also plan on integrating data from wearable medical devices on our platform with AI guided real time interpretation and recommendation.

We offer free and premium packages to caregivers with our dedicated features accessible to premium subscribers at $55.56/mo & $138.92/mo.
We plan to leverage the community of our platform to run advertising services for pharmaceutical and medical devices firms at $27.78/week
We plan to provide relevant data on our platform to interested corporate bodies for a fee.
Why we are positioned for long-term sustainability, profitability, expansion: By positioning our platform as the main aggregator of health data, we plan on securing partnership with key stakeholders like the World Health Organization (currently completing the process), then leveraging on this partnership to expand our reach to health organizations. As the number of subscribers increase, so does the value of advertising on our platform and the quality of data increase.
We are applying to solve because we share a common commitment of advancing health using basic technology.
We believe Solve can advance our work by connecting us to strategic partners like internet service providers to power internet access for frontline health workers on our platform and health governing bodies like the World Health Organization and the ministry of health which benefits from our platform by having access to realtime health and disease surveillance data.
Barriers: 1. Poor internet access 2. Power unavailability 3. Cyber phobia 4. Lack of will 5. Decentralized Electronic Health Records solutions
Solve can help us overcome these barriers by connecting us to strategic internet service providers like MTN Group which also offers renewable energy services via MTN Mobile Energy (Lumos); global health bodies like the World Health Organization and the World Bank who can facilitate the adoption of a central health records system by governments.
