1. NanoHealth is addressing one of the biggest health care challenge of our generation- rising burden of chronic conditions. NCDs account for an annual loss of 5-10% of GDP in India and more than 60% of all deaths in India, leading to a crippling effect on the individual and the economy. Prevalence rates of diabetes increased 135% in the last 10 years in low income groups compared to 24% rise in middle income groups. NanoHealth combats the key drivers of the growing NCD burden in India- under-diagnosis and care fragmentation in managing them. NanoHealth is a technology driven human touch based chronic disease management solution that integrates with the existing health systems (Public and Private) to make them proactive and continuous.
NanoHealth solution addresses the systematic problem of our current health systems with a focus on patient and health outcomes. The most innovative part is Recommendation Engine that analyzes all the data around patient’s health and provides proactive recommendation to the patients and health coaches.
2. The NanoHealth solution focuses on early diagnosis and continuous care in a seamless fashion to ensure better health outcomes. Technology guided Health Coaches become primary points of contact that aid in better health behavior and care-coordination for patients. The technology platform provides appropriate personalized views to entire care team (physicians, specialists, diagnosticians, nutritionists), notifies them proactively to provide a coordinated care. The solution completes a full circle of health with patients at its center: Screening (Health Coach)->Diagnosis (Doctor)->Prescription(Doctor)->Compliance measurement(Patient)->Monitoring and Care coordination(Health Coach)->Health Outcomes (Technology)->Prescription changes(Doctor)-> Repeat.
The solution is being used by the public health system for screening and disease management at scale, increasing the efficiency and effectiveness of existing health systems. Leveraging the platforms, for the first time in India, Health analytics is being used to design and implement Employee Wellness Programs and track employee health on a dedicated dashboard.
3. Impact: We measured the effectiveness of the program in improving health of the users by its ability of (i) Case Finding- find undiagnosed and undertreated cases by its screening activities, and (ii) Case Management- improved and controlled health measurements (blood glucose and blood pressure) among enrolled users.
Case Finding: 59% (25,241) were found to have uncontrolled systolic blood pressure and hence at risk of developing hypertension or its complications; 28%(12,190) had uncontrolled fasting or random blood glucose levels and hence at risk of developing diabetes or its complications. Of those who did not have previous history of diabetes, 21% were found to be at risk of diabetes and 65% were found to be at risk of hypertension. These were the newly diagnosed users for developing diabetes/hypertension identified by health workers. Further, 17% of those without any previous history were found to be pre-diabetic and 36% were found to be at risk of pre-hypertension.
Case Management: We found a significant decrease in fasting blood glucose by 20.1mg/dL and systolic blood pressure by 9.2 mmHg compared to their baseline values after twelve months among users who stayed on the plan for 12 months.

The NanoHealth integrated technology platform enables proactive care process and ensures care coordination through three key features:
1. Every interaction of the user with the health system across all providers is captured, and relevant providers access this information to ensure care continuity. The platform also supports the health workers and other providers to perform their tasks effectively.
2. Individuals have access to their health information on digital platform enabling them to be more aware of their health status
3. A built in 'Recommendation Engine' that analyses individual health data and provides personalized recommendations to mitigate the risk of disease progression.
NanoHealth entails a unique blend of human touch and technology to provide integrated healthcare management to communities. It delivers proactive, continuous and coordinated care which is not practiced in today’s health systems. The most innovative part is Recommendation Engine that analyzes patient’s health data and provides proactive recommendations to patients and health coaches. The core platform of technology and human touch is flexible enough to work different settings like for governments, primary clinics, corporates, health and wellness programmes.
The entire tech platform sits with the existing health system and integrates with it which makes the health system efficient and effective.
We plan to expand our solution to provide end-to-end management for three additional chronic conditions which includes Chronic Kidney Disease (CKD), Chronic obstructive pulmonary disease (COPD) and Mental illness. We also aim to expand our solution to cover 100,000 people in next 12 months.
NanoHealth will focus on providing training and technology and this solution can be implemented anywhere in the world. The problem of NCD is alike both in developed and developing countries.
Over next 3-5 years, we plan to expand our solution to developing countries in East and Southeast Asia.
Rigorous evidence on impact and cost effectiveness of large scale NCD management programs, such as NanoHealth, will be critical for governments to invest into them. Currently, such evidence does not exist in India. NanoHealth will target 1 million people in next 3-5 years for providing end-to-end management in collaboration with Govt. of Telangana, India.

NanoHealth solution addresses all the identified gaps in the current healthcare system by:
Early Detection: Ensure active screening by front line health workers (Saathis)
Clinical care coordination: Ensure care coordination between various levels of providers through the NanoHealth technology platform.
Continuous Care Management: Deliver continuous care management for high risk patients.
The beneficiaries are retained because of improved health outcomes as evident by reduction in BMI, Blood Sugar, BP levels and reduction in tertiary care visits, hospitalizations.
We are actively serving 70,000 people in managing their health with our solution. The beneficiaries are approached through front line health workers who are their central point of contact to avail all the health services at their doorstep.
The impact is evidenced by reduction in fasting blood sugars by 1.59 mg/dl and reduction in systolic and diastolic blood pressure by 0.27 mmHg, and 0.20 mmHg respectively for each health worker (Saathi) visit from the time of first screening amongst NanoHealth beneficiaries (p value< 0.05). We aim to target 1 Lakh population in next 12 months in collaboration with Government of Telangana. In next 3 years, we would like to extrapolate this to half a million underserved population. We aim to achieve the following,
1. Understand the care seeking pattern of individuals with chronic conditions
2. Test operational and financial viability of implementing NCD management solution in public health system
3. Measure the impact of the NCD management solution on increased use of public health systems, improved health outcomes and improved beneficiary satisfaction.
The impact of the solution would be first visible in 6 months following initial screening.

NanoHealth has chosen to prioritize partnering with public payers in India in order to serve the underserved at a large scale. The National Health Policy of India, 2017 has set a goal to establish “controlled disease status” for 80% of known diabetic and hypertensive population by 2025, making a compelling case for public investment into NCDs.
NanoHealth intends to work with one such public payer, the State Government of Telangana. NanoHealth aims to partner with Govt. of Telangana to provide NCD management program to underserved beneficiaries. Operationally, NanoHealth will integrate into current Govt. system by plugging our technology platform onto the existing institutions and stakeholders.
Integrated Health Solution does 2 things - First, it identifies the disease early in the people so that people come early into the system which adds value to the healthcare system and Second, all the efforts which goes into the disease management due to which the compliance is higher. Since the compliance is higher, it ensures more health care resources are utilised and the person lives longer which means the revenue increases for all the stakeholders in the health care system.
