The care team portal enables community workers and clinicians remotely monitor and deliver care virtually.
Only 12% of the US population is health literate which makes majority not known to medical jargon or know how to obtain information on managing their health. This contributes to the $528B challenge of non-optimized medication therapies. Folks struggling with Substance Use Disorder have more challenges given how the opiate takes over one’s cognitive abilities. A toolbox approach is needed which combines best of breed practices like 12-step, mindfulness and more. Close to 50% of SUD patients take medications for a chronic condition and majority have a history of mental illness. Given health literacy levels in the country, aging and chronic condition population, we have honed in on truly simplifying, automating and being a digital pharmacist to enable better efficacy and wellness. Our offering makes patient feels comfortable in taking their meds because of the safety checks and schedule medication we conduct, improves patient compliance and support care team provide that needed support network during the current pandemic remotely.
AI-based medication timing suggestion is a key technology for us, along with AI-based suggestions on what to do next based on reported symptom in terms of mental health. The clinical intelligence and guided approach to enable self-care at home, the comprehensive and holistic nature of the app where it offers a meditation session if someone reports high anxiety levels and the closed loop via a Clinician portal for communication to take place. Educational materials and other updates can be pushed out to patients, along with symptom monitoring. Our approach is to empower patients with self-care tools to know when to do something, what to do and how.
