NanoHealth
71% of all deaths today are because of Non-Communicable diseases (NCD), 85% of them in developing countries. The rising problem of NCD cannot be solved by current reactive and fragmented health systems.
NanoHealth offers a unique combination of human touch and technology to provide personalized care management, care-coordination across providers and medication adherence support to improve health outcomes. Technology platform ensures scalability without compromising on the quality of care. NanoHealth’s solution gives a personalized view of each of the stakeholders to effectively do their job with absolute focus on patient’s health. This solution combines with existing healthcare stakeholders & infrastructure, making it efficient and effective. For providers, clinics and hospitals, it increases patient engagement and patient outcomes. Health analytics allows to identify the people who are very sick and then tools to engage them in providing the best care.
According to WHO, 39.5 million of 56.4 million global deaths in 2015 were due to non-communicable diseases (NCDs)such as cardiovascular diseases, cancers, diabetes, and chronic lung diseases. In India, NCDs account for 61% of the morbidity and mortality, this is rising disproportionately in the low-income segments. Current health systems(both public and private)are designed primarily to address episodic/acute complications arising out of these conditions rather than managing these conditions over the long term to avoid such complications. The need for the hour is for interventions that can support in improving the diagnosis and management of NCDs. Fortunately all these diseases can be prevented and it can be reversed.
NanoHealth works at scale with communities to provide proactive and coordinated care leveraging its technology and health workers. NanoHealth works with bottom of pyramid along with government and large donors to screen, diagnose and manage long term diseases for people. Currently, we are going to start to work in district town of Telangana state of India to provide an integrated solution from community screening, primary care to tertiary care.
NanoHealth also works with Corporates to manage their employee’s health. Apart from showing trend of improving health of employees, NanoHealth has been able to show reduction in the claims of the enterprise.
NanoHealth’s technology platform makes the existing health system proactive, coordinated and continuous by providing the right information to all the stakeholders at the right time. Unlike communicable diseases like plague, malaria etc that can be treated within a few days, Non-communicable diseases cannot be treated, it can only be managed. Hence, the solution to address the problem of NCDs are very different from the way communicable diseases are managed and doesn’t lie only in the hospitals and clinics. It requires a lot of user engagement over a long period of time.
NanoHealth utilizes its proprietary recommendation engine to ensure care-coordination across various stakeholders while capturing each and every activity in the health system. The engine comes up with actionable next steps for every activity considering both clinical and operational parameters. It starts with a simple screening that captures demographic, clinical history and health parameters. Based on the captured data, risk profile is generated and the user is recommended to meet General Practitioner, specialist or nutritionist. Once the diagnosis is confirmed,
As per the study conducted on more than 50,000 people reached out so far, it was found that NanoHealth was successful in identifying undiagnosed and under treated population at risk of developing diabetes, hypertension, or its complications. Twenty eight percent of the screened individuals were at risk of developing diabetes or its complications; 21% of who had no previous history/knowledge about the disease. Fifty nine percent of the screened individuals were at risk for developing hypertension or its complications;65% of whom had no previous history/ knowledge of the disease. Individuals, who enrolled on the NanoHealth plan and continued it for 12 months, experienced a significant decrease in fasting blood glucose (20.1mg/dL)and systolic blood pressure (9.2mmHg)compared to their baseline values.
- Reduce the incidence of NCDs from air pollution, lack of exercise, or unhealthy food
- Enable equitable access to affordable and effective health services
- Growth
- New business model or process
While the whole world is trying different approaches to solve the NCD problem including awareness, incentives etc., we have been able to show a proven, scalable method of addressing the problem. Unique and seamless combination of technology and human touch across the health value chain is a big differentiator for the solution. NanoHealth’s configurable recommendation engine that ensures care-coordination is the most impactful innovation of the solution. Strong health analytics to identify who are the patients that needs maximum care and provide them with the best care in the most convenient ways are another few examples of the solution being innovative and effective.
The NanoHealth integrated technology platform enables proactive care process and ensures care coordination through three key features:
1. Every interaction of the user with the health system across all providers is captured, and relevant providers access this information to ensure care continuity. The platform also supports the health workers and other providers to perform their tasks effectively.
2. Individuals have access to their health information on digital platform enabling them to be more aware of their health status
3. A built in 'Recommendation Engine' that analyses individual health data and provides personalized recommendations to mitigate the risk of disease progression.
The entire tech platform fits with the existing health system and integrates with it which makes the health system efficient and effective.
NanoHealth’s solution ensures timely patient engagement. It helps risk profile people from the community so that care can provided to them timely. There are a lot of inbuilt tools that can be used to engage with the patients in a very personalized manner.
Apart from that the solution has already proved that individuals, who enrolled on the NanoHealth plan and continued it for 12 months, experienced a significant decrease in fasting blood glucose (20.1mg/dL)and systolic blood pressure (9.2mmHg)compared to their baseline values. We are actively serving 70,000 people in managing their health with our solution. The beneficiaries are approached through front line health workers who are their central point of contact to avail all the health services at their doorstep.
We aim to target 1 Lakh population in next 12 months in collaboration with Government of Telangana. In next 3 years, we would like to extrapolate this to half a million underserved population. We aim to achieve the following,
1. Understand the care seeking pattern of individuals with chronic conditions
2. Test operational and financial viability of implementing NCD management solution in public health system
3. Measure the impact of the NCD management solution on increased use of public health systems, improved health outcomes and improved beneficiary satisfaction.
The impact of the solution would be first visible in 6 months following initial screening.
We plan to expand our solution to provide end-to-end management for
three additional chronic conditions which includes Chronic Kidney
Disease (CKD), Chronic obstructive pulmonary disease (COPD) and Mental
illness. We also aim to expand our solution to cover 100,000 people in
next 12 months.
NanoHealth will focus on providing training and technology and this solution can be implemented anywhere in the world. The problem of NCD is alike both in developed and developing countries.
Over next 3-5 years, we plan to expand our solution to developing countries in East and Southeast Asia.
Rigorous evidence on impact and cost effectiveness of large scale NCD management programs, such as NanoHealth, will be critical for governments to invest into them. Currently, such evidence does not exist in India. NanoHealth will target 1 million people in next 3-5 years for providing end-to-end management in collaboration with Govt. of Telangana, India.
Need Vs Demand: Even though a significant number of people are dying out of NCDs, the maturity of the people to accept and pay for preventive solutions are still limited. Though the government has already committed to focus on the NCDs, decision making is usually slow. So, even though there is a glaring need for an effective NCD solution, demand is still in its nascent stage.
We have been able to prove that even for B2C segment in slums, people can pay after the trust between the health worker and the community is established. It takes 12-18 months for a health worker for financial break-even. Any financial support to to expand for this time duration will be very helpful as well.
To solve the problem of demand, we plan to focus on marketing the results of our work so far. Testimonials of patients who have been actively able to manage their health through the support of our solution is one such example.
Also, pitching the impact to leverage corporate CSR, and large Donors to showcase measurable impact will be key to scale the solution to newer areas.
